Title: Can hospices be integral to 24/7 care?

Outline: With many hospices facing financial challenge, and within a struggling health care system; (how) can hospices be responsive, integral partners to a 24/7 care system? Discussion of opportunities and challenges. Using a case study from Yorkshire, we will describe and discuss examples of system-wide collaborative innovation to optimise the use of hospice beds, whilst supporting the avoidance of hospital admission. This will include: models to facilitate consistent 7/7 hospice admitting capacity; direct access to Palliative Medicine consultants for paramedics out of hours; and rapid transfer of actively dying patients from Emergency Departments into hospice beds.

This session will be lead by Dr Hannah Zacharias, St Gemma’s Hospice.
