As young people with life-limiting conditions are living longer we are increasingly likely to meet them in adult palliative care – either as part of their planned transition to adult services or, more frequently, in their early adulthood, when they face issues with symptom management, complex decisions around treatment limitation, or at the end of life. The Transition and Young Adult special interest forum is a safe space for palliative care professionals to share our experiences and the things we are learning from working with this age-group.
We are a cross-organisation group, partnered with the Association of Paediatric Palliative Medicine, and include adult and paediatric medical and nursing members from across the UK and Ireland. We offer peer support and learning through quarterly teams meetings and discussion on our NHS Futures platform: Associations of Palliative Medicine and Paediatric Palliative Medicine Transition and Young Adult Special Interest Forum – Futures
